The paperwork members ask for most, grouped by what you are trying to do. Most can be signed electronically and submitted without printing anything.

Membership applications, name and address changes, joint owner additions, beneficiary designations, and account closure requests.

Loan applications, payoff quote requests, skip-a-payment, insurance verification, title release, and hardship or deferment requests.

Outgoing wire requests, recurring transfer authorizations, direct deposit setup, and external account linking.

Unauthorized transaction claims, billing error disputes, ACH stop payments, check stop payments, and merchant chargeback statements.

How to get a form back to us
Secure message is fastest and safest. Email is the one channel we ask you not to use for documents with account numbers on them.
Open the form
Each PDF is fillable on a phone or a computer. Nothing needs to be printed unless the form specifically calls for a notarized signature.
Sign it
Most forms accept an electronic signature. Estate documents, notarized affidavits, and safe deposit changes still require a wet signature in a branch.
Send it securely
Attach it to a secure message in online banking, hand it to any branch, or mail it to P.O. Box 4820, Cedar Ridge, MN 55104.
Watch for confirmation
We acknowledge receipt within one business day and tell you if anything is missing, so a form never sits in a queue waiting on a signature you did not know about.

Common questions
Can I email a completed form?
We ask that you do not. Ordinary email is unencrypted and these forms carry account numbers. Secure message inside online banking takes the same effort and protects the document.
Which forms still need a notary?
Small estate affidavits, power of attorney acceptance, and certain trust certifications. Every Summit branch offers free notary service to members.
Do I need to print anything?
Rarely. The PDFs are fillable and most accept an electronic signature. Printing is only necessary when a notarized or witnessed signature is required.
How do I get a copy of a disclosure or the fee schedule?
All current disclosures and the full fee schedule are available in the disclosures group above, and archived versions can be requested from member service.
Can someone else submit a form on my behalf?
Only with a power of attorney we have on file, or as a joint owner on the account. We will ask for identification and documentation before acting on a third-party request.
The form I need is not listed. What do I do?
Call member service at 800-555-0100 and describe what you are trying to accomplish. In many cases there is no form at all and the request can be handled on the call.
